Patient Forms
As with any doctor’s office and seeing a new doctor you will be asked to fill out some new patient intake forms. This information provides the Doctor with the necessary information to help you and better understand what is happening in your body. Click here to download the patient paperwork.
Face to Face
Once your forms have been completed, you will have a one on one consultation with the Dr. Popkin to discuss your health-related problems and issues. A thorough history consisting of a physical exam plus a chiropractic, orthopedic and neurological exam will be conducted to determine what exactly is causing your health demise.
X-rays
If you require X-rays our facility has the ability to take the necessary X-rays on site.
Treatment Right Now!
If Dr. Popkin finds it necessary to administer treatment due to a patient being in pain or discomfort this will be performed immediately. In the past Dr Popkin has performed physical or physiotherapy in the form of EMS, Heat Therapy, Ice Therapy, Ultrasound , Paraffin Wax, Mechanical traction, Acupuncture or Massage therapy.
Home Care or Recommendations
If the patient needs specific home care instructions this will be reviewed prior to leaving our office. Specific instructions will be given to you concerning whether to use ice or heat at home, how to sleep,how to sit properly and how to properly do activities of daily living such as sleeping , bending, brushing your teeth, getting in and out of a car and showering. This may also include what not to do at home or work to further injure yourself.
As You Exit
Upon leaving and feeling better you will be given a follow up appointment to go over your exam findings and x-rays.
